Oakwood Village offers programs for
older adults
Those living or working at Oakwood know that our community does much more than provide housing to older adults. Oakwood promotes
and supports healthy lifestyles, and our focus on this continues to increase.
Simply put, all too often we see people come to Oakwood's nursing home and assisted living facilities with ailments that could have been
prevented through education and a proactive approach to wellness. Are older adults really comfortable leaving their health to fate or, if
given the resources, do they want to exercise some control over the way they age? By using some resources now to determine their health
risks and putting a plan in place to reduce them, seniors can decrease the chances of unnecessarily needing skilled nursing or assisted living
care.
Enriched Care is designed for people who want to proactively address concerns about their health and aging and positively impact their
health, rather than simply wait for an eventual decline. They are supported by a compassionate, understanding, physician-led team of
Oakwood professionals. Enriched Care is designed to provide older adults with the knowledge, resources and support they need to achieve
their highest level of well-being.
The purpose of this program is to assess the health of participants, educate them on the strengths and limitations relating to their overall
health, and create a comprehensive personal health plan designed to increase the chances of participants maintaining their independence
and reaching a higher level of personal wellness. Participants in Enriched Care are involved in creating a personal health plan focused on
reducing their health risks. The personal health plan is formed using the latest information in geriatric medical research and the input of a
team of Oakwood professionals armed with years of experience and training working with older adults.
The program begins with the participant engaging in an initial assessment — a series of tests specially designed to help gain a complete
understanding of his/her health status and the risk factors they may face as they age. Then, the participant meets with the Oakwood team to
gain a full understanding of his/her health, and together, they develop a personal health plan geared towards maintaining or improving the
specific health concerns of the participant.
The key is that the participant drives this process. While participants are provided with information, resources and support, they are not
"told what to do." They work with a medical director specially trained to work with older adults and with a team of Oakwood professionals to
create their own individualized personal health plan. The team takes the time to share the outcomes of the assessment with the participant,
explains the participant’s overall strengths and general health’s limitations, and discusses the strategies and options available to help
achieve the participant’s highest level of well-being.
From this point, the program is designed to remain flexible in order to meet the needs and address the specific assessment results of each
participant. Participants are encouraged to use Oakwood Village staff as resources to implement their plan of care. Oakwood has the
majority of the resources participants may need in order to engage in their care plan, including consultation with a registered nurse,
physical therapist, occupational therapist, registered dietician, chaplain, certified personal trainer, social worker and pharmacist, as well as
access to a medical doctor specially trained to work with older adults. These specialists can meet individually with participants, as needed,
in order to guide them through the behavior changes necessary to improve their chances of healthy aging and to educate them further on
their health limitations and options for improvement on an individual basis.
Enriched Care is designed to complement, not replace, the participant's relationship with their personal physician. The involvement of a
primary care physician is critical to the participant's well-being and we will consult personal physicians throughout the program as needed to
find a comprehensive, integrated and holistic approach to eliminate the negative side effects that can result from simultaneous treatments
of separate medical conditions.
Oakwood is thrilled to offer this innovative new program and is uniquely positioned due to its comprehensive staffing, the number of
services it offers, and its close proximity and strong relationship with the University of Wisconsin-Madison School of Medicine and Public
Health, Div. of Geriatrics, to do more to promote healthy living. We are confident Enriched Care can help seniors reach a higher level of
personal wellness by bringing the skills, experience, and real hope to support independence for as long as possible.
Enriched Care was designed by a team of Oakwood Village professionals in collaboration with Steven Barczi, MD, associate professor at
the UW-Madison School of Medicine and Public Health, Div. of Geriatrics. This program is available to older adults in the community as

Enriched Care from Oakwood Village offers older adults an individualized, proactive approach to
healthy aging.

What inspires you as you grow older? What motivates you each and every day?
The answer is different for everyone, depending upon your background and current health. For
some, inspiration comes from volunteering in the community — making the world a better place,
one day at a time. Others can't wait for their next visit with their grandchildren, a great excuse to
laugh and play. For others, their goal may simply be to remain in their homes, the place where
they have lived for many years and want to remain for as long as possible.
Oakwood Village Retirement Communities has served seniors in Madison for 60 years,
establishing a strong reputation for compassionate and excellent service. The people who work at
Oakwood believe strongly that every person is of infinite worth, and should be enabled and
assisted to attain or maintain their highest level of well-being. This is the philosophy that
motivates us to serve our residents each and every day.
This is also the inspiration behind the Oakwood Village Enriched Care Program. As we
continue to care for seniors, we have accumulated a wealth of knowledge about the needs and
risk factors of older adults.
